There are plans to move Hawthorn Surgery to be part of a multi-use community building at the site of the existing community centre in Greenstead, alongside other services which support the health and wellbeing of local people.
A survey about this is now open and includes questions about the service you currently receive from us at Hawthorn Surgery and any additional health services you would like to see provided at the new surgery.
The survey is open until Sunday 17 December 2023.
The survey website explains the aims of the whole programme and lists local engagement events where paper copies of the survey can be filled in. Paper copies are also available at Hawthorn Surgery.
